Iron is essential for dopamine synthesis. Without adequate iron your brain cannot produce dopamine efficiently regardless of what medication you take. A ferritin level that your doctor calls "normal" might still be too low to support optimal neurotransmitter production. The clinical range for ferritin is broad. A result of 15 ng/mL is technically within range but it is far below the 50 to 70 ng/mL level where most people feel their best.

The overlap between iron deficiency and ADHD symptoms is striking. Difficulty concentrating. Restlessness. Impaired working memory. Fatigue that caffeine does not fix. A study published in Archives of Pediatrics found that children with ADHD had significantly lower serum ferritin levels than controls. More importantly, the severity of ADHD symptoms correlated directly with how low the iron was.

This does not mean ADHD is caused by iron deficiency. It means that iron deficiency can amplify existing ADHD symptoms dramatically and it can also create ADHD-like symptoms in people who do not have the condition at all. Either way, checking your ferritin is one of the simplest and most informative blood tests you can request.

Stimulant medication requires dopamine to work. If your body lacks the raw materials to produce dopamine, the medication has less to work with. People with low ferritin frequently report that their Adderall or Vyvanse "stopped working" or that the effects feel weaker than they used to. Correcting the iron deficiency often restores medication effectiveness without needing a dose increase.

The Science

Ask your doctor specifically for a ferritin test, not just a standard iron panel. Ferritin measures your stored iron and is a far more accurate indicator of your functional iron status. If your result is below 50 ng/mL, supplementation may be worth discussing. Iron bisglycinate is the best tolerated form and does not cause the constipation that ferrous sulphate is known for. Take it on an empty stomach with vitamin C to maximise absorption. Do not take iron within two hours of your stimulant medication or any calcium-containing food as both interfere with uptake.
